BACKGROUND

This item, on behalf of the Office of Community Care and Empowerment, authorizes an amendment to an existing lease agreement with Martin Luther King, Jr. Family Health Clinic doing business as Foremost Family Health Centers ("Foremost") for and additional five years for approximately 25,247 square feet of office space located at 2922 Martin Luther King, Jr. Boulevard, Building B. The five-year extension will provide for the continued use for a community-based family health and wellness center that is accessible and affordable for the insured, uninsured or underinsured patients of all ages.

Foremost is a community-based health center which opened its doors for service in August 1986 and has been operating at the Martin Luther King, Jr. Community Center since July 2002. At that time, the center had one pediatrician, two nurse practitioners, two dentists, a lab and pharmacy services. In the early 1990's, the center added an immunization clinic and a mobile unit which served children throughout Dallas.
